• Living alone
• Lonely
• Isolated
• Housebound
• Recently Bereaved

The Practice can refer you to the Durham Alliance for Cocmmunity Care (DACC).

Their aim is to help you stay as independent as possible. Reducing loneliness is a part of this.

Once you are refrred their team will be in touch to arrange an appointment to call and see you.

During their visit they will try to ensure that your needs are met and you are warm, fed and safe.

To book an appontment, ask Reception to refer you to the Home Visitor Service or call Durham Alliance for Community Care (DACC) on 0191 384 3679.
